New to CMA?

We're glad you're here. New patients are always welcome across all our specialties. We work to get you scheduled promptly — typically within a few days for primary care and 1-2 weeks for specialist appointments.

To make your first visit as smooth as possible, please review what to bring and what to expect. If you have questions before your appointment, call us at (325) 000-0000.

Arriving 15 minutes early gives you time to complete new patient paperwork without feeling rushed. You can also download and complete forms in advance through our patient portal.

For Specialist Visits

Referral from your primary care physician (if required)
